PHAR 5149 Medication Therapy Management
This course teaches students how to perform Medication Therapy Management (MTM) in a variety of pharmacy settings building on the cumulative disease and topic knowledge and communication skills taught throughout the curriculum to date. Students will learn and practice the five core elements of MTM: conduct medication therapy reviews; write patient language medication lists and action plans; detect and solve medication related problems; conduct prescriber communication interventions, and complete documentation and follow-up.
Prerequisite
Pharmacy students enrolled in the third professional year of study will be eligible to take this course.
